If West leads a small D not the 9 then East has to get it right. Easy seeing all 4 hands of course.

On reflection, and no question, East should insert the ten, but so early in the hand and usually only a yawn from East and routine duck, but instead, my hat would always come off my head (although I suspect I have hardly ever worn one) if anyone is quick witted enough to so quickly, read declarer’s mind, especially before he has yet to see it occur.
However, written in the rules, East is allowed a little extra time at trick one, allowing him a fighting chance to be brilliant. In any event, it is certainly worth, at the very least, dreaming about.
