I, too, presume East played an honor on the first diamond led from dummy.
Of course, a heart switch is difficult to imagine from East but not s a club, since West’s bid and South’s 3NT bid and early play likely showing the king of spades. should have, at least IMO, been enough for East to now switch to a club, denying the declarer a make.
